We welcome referrals from all medical and allied health professionals, and with your patient's consent, we can keep you updated on their plan and progress.
How to refer
Email your referral to info@gutbraindiet.com.au. Please include:

Patient name, date of birth and contact details

Reason for referral and relevant history

Current medications

Recent relevant pathology or imaging

Whether a Chronic Condition Management plan is in place, and the number of dietetics sessions referred
GP Chronic Condition Management Plans (GP CCMP)
Patients with a GP CCM plan and a dietetics referral can claim a Medicare rebate of $63.40 for up to 5 allied health sessions per calendar year. We can process eligible claims on the patient's behalf at the time of consultation.
What you'll receive
A written report after the initial consultation and at the end of any program, plus an update whenever something needs your attention.
